Required Qualifications

Social Worker or Case Manager experience

To coordinate and be a spokesperson for HIV Prevention 

Serve as the lead to address social determinants of health's barriers of care

and connecting resources to meet identified needs with a focus on transportation, food, housing and finance.

Licensed MSW or RN


Description & Duties

1.    Lead the HIV prevention promotion, outreach and communications strategies to support program goals and increase community awareness.

2.    Ensure that HIV prevention services is maintained ongoing efforts.

3.    Experience with excel, word and EMR reporting and documentation.

4.    Provide timely updates to Managers and document intervention and program outcomes according to an established workplan.

5.    Prepare reports to supervisor and funder, as per established reporting schedule.

6.    Work with Managers to eliminate barriers to HIV testing, prevention and SDOH screening.

7.    Build community partnerships

8.    Manages referrals for follow-up of positive testing and screening.

9.    Works directly with patients, clinical staff to connect patients with needed resources.


Other info

1.    Become trained in all relevant CDC-endorsed evidence-based interventions currently implemented by HIV prevention programs.

2.    Become a Certified Application Counselor and serves as the lead for Marketplace enrollment.
